How come my warrior can see other civ's units when they are miles away!!!! I mean it's ancient era!!! no GPS or satellite during ancient era!!!! wth weird!!!
Or you could be standing on a hill just outside a city's border. That gives you better sight as long as there are no hills, mountains or forests/jungles in the way.
technically, it's that you're controlling the holy city and their city has your state religion in it. which means this works if isabella founded the religion and then you eliminated her and moved in the city changed hands. you do have to be using that religion at the time. you get the line of sight into their cities with your religion whether they have it as their state religion or not. so you can (with OB and if they're not in theocracy) spread your religion to their city to get pseudo-spying if you want to.
i once met the russians for the first time when my state religion spread to cathy naturally over the ocean after i knew astronomy, and i had the holy city of Barcelona. i didn't even remember that there was a civilization i hadn't met yet, and it scared me! i was not playing as Isabella and won't say how i came to acquire Barcelona .
If a foreign city (no matter how far away it is) adopts the same religion as you (and you founded the religion - I think), then you can see that city.

Well, the people of the city are visiting the Holy City of their religion, which is located in your empire. In your Holy City they are talking about the city they came from, and your secret agents listen to them and bring that intelligence to you.

Well, it is not a lone warrior. It is a unit of warriors, and they have a scout or two in every unit, which examines the terrain around them, sometimes travelling miles from their camp. And it is much more easy to examine the terrain from hills, so units on hills gain an increased line of sight.
