Wrong terrain art applied (mismatch between terrain and art)

Something is causing the wrong terrain art to be applied to certain tiles. Recently I've been in two single-player games where this bug occurred.

The terrain is correct in the Strategic View, and on mouse-over, but it is displayed incorrectly in the Normal Game View.

In particular, this bug seems to affect mountain tiles (which have non-mountain art) and non-mountain tiles (which have mountain art). So maybe it's a mountain-specific thing. Or maybe it's just by coincidence that this is the only place I've seen it. EDIT: I have also seen Antiquity Site art applied when it shouldn't have been. I'm not sure if that's this same bug, or if it's a bug where Antiquity Sites are revealed early.

I am attaching a save to demonstrate. Between Delhi and Persepolis, there is a range of mountains. However, there is also a plains tile with horses that is displayed as horses on top of a mountain in Normal Game View. Two tiles to the northeast, there is a mountain tile that is displayed as desert, hills.

I saw something similar recently, a tile that looked like grassland but was designated as a mountain tile and unsurpassable by units.
 
Here is another example save that shows this bug occurring. It's the 3rd game where I've noticed it.

Between Yerevan and Madrid, there are four bugged tiles.
  • One is Jungle, Hill, but shows as Jungle on top of a Mountain.
  • Another is Wheat, Plains, but shows as Wheat on top of a Mountain.
  • Another is Sugar, Marsh, but shows as Sugar on top of a Mountain.
  • Another is just Plains, but shows as a Mountain.
There is also one bugged tile northeast of Sidon.
  • It's Snow, but shows as Snow, Hill. This is the first one I've noticed that doesn't involve a Mountain, one way or the other. But it's a Hill. Maybe it's a problem with "elevation" art?

It just occurred to me that I think I've only noticed this bug when playing Assyria. Maybe it's related?

I am currently playing a game where Antiquity Site art was misapplied. I hadn't researched Archaeology when the art was applied to a tile, but when I did research Archaeology, there really was a Site there. So maybe this is the same art-mismatch bug, or maybe it's an early-Antiquity-Site-reveal bug.
 
In all previous instances of this bug, fake "Mountains" (non-Mountain tiles with Mountain art) were passable as if the Mountain did not exist.

However, in the attached save game, there are two "Mountain" tiles that are not passable. One is Fallout, Desert, and one is Fallout, Plains. The tooltip and Strategic View show them correctly, but they have Mountain art, and are not passable.

I think this may be caused by the fact that there is Fallout on these tiles. I hate to say it, but I didn't notice before the bomb went off whether the Mountain art was there, and whether the tiles were passable. But that's the only thing that's obviously different from other instances of this bug.

Here is another instance of this happening. This is a 2 player hot-seat game, version 1.0.3.144 w/G&K+BNW. Between Tikal and Chichen Itza, there's an atlatlist standing on a "mountain". Also a couple of "hills" that are actually flat, near Palenque.
