I got a hypothetical one...
Specialist Economy in an 18-civ world... all the other 17 civs switch to Emancipation. =D
Even the infrastructure + culture slider remedy won't be enough to deal with that.
Here's a legitimate one, though...
Founding Buddhism on a huge map, and then Saladin spreads Hinduism to EVERYBODY (minus 1 who was too far away for his missionaries to make it alive).
Even a civ with its capital converted to Buddhism mysteriously switches to Hinduism the moment a minor city is converted.

My biggest fail was taking a peacevassal while in a permanent alliance. I had already thought I learned my lesson on this, but they guy was so pitiful and supplicative that my resolution failed. So then, the permanent ally gifts the peacevassal our entire tech lead, and the peacevassal sells it to everyone else on the map. I guess this wasn't a truly epic fail, but the fact that it was the same egregious mistake, TWICE, was, well... really bad for me. The only good thing about this was that it caused me to spend a lot less time in front of the computer for a while.
