Cities are losing tile output

Question 1: My problem is that my cities are losing output from its adjacent tiles, until a point where they only have output from the actual city tile. I.E: Only 1 tiles gives me production and food. I have been constantly losing output now, and my cities are down to level 7 now. My adjacent tiles are improved.

Question 2: I am trying to achieve cultural victory, and have only 2 cities. Hence, I own many tiles aroung my city. Is there a maximum number of tiles that produce output for my cities?
 
Cities can only work tiles out to three hexes outside the city itself, regardless of how many additional tiles outside that radius have been claimed via culture.

I'm not sure what your problem with losing production is... a screenshot might help. Maybe you've accidentally clicked on tiles in the city screen, which has locked those tiles out, preventing them from being worked.
 
Sounds like your cities do not get enough food and starve.

Remember:
You can only work one tile per pop point of a city, so even if you improve everything around it, only some will be worked by your citizen!
The tile yields shown on the map (toggle by pressing "y") only show the POTENTIAL yield, not if it is worked or not.

Please upload a screenshot from your city view (opened by left clicking on the city name).

I know it's often better to ask a dumb question instead of keeping silent forever.
But sometimes it really helps to use your own imagination. ;)

Have you assigned lots of specialists, or has the city governor AI assigned them automatically, leading to fewer citizens working tiles?

A screenshot would help a lot :) .
