Can volcanoes or floods kill settlers? Or perhaps they ran unprotected into a barb?

That's probably what has happened, floods indeed can kill settlers and I've seen a post or two on reddit illustrating just that, when the player is wiped out on turn 1 or 2.floods kill settlers
They did take their first steps... off a cliff. From the first stirrings of life beneath water, to the great beasts of the stone age, to man taking his first upright steps, you have come fa... And you dead.
I find it so very satisfying to settle a canal city like that.Canal city by Germany, nice!
Despite I set up the map as Continents, the game generated a single long continent where everyone besides me was on. And it is actually better than anything the Pangea map script would generate...
This even happens in real life. This happens sometimes, where two large landmasses are connected by a small bridge of land. Pretty cool when it does.
I once played a game where every player was Cyrus. Led to a lot of funny notifications, highlights being:
"Rumor has it that Persia has captured Pasargadae."
"Cyrus has been defeated by Cyrus."
And watching as Cyrus denounced, declared war, and made friends with himself. Additionally, declaring a surprise war pretty much went as follows:
Which just led to ideas like "Oh, you know, Cyrus loves it when you declare surprise war on Cyrus, but hates it when you declare war on Cyrus."
- The Cyrus you attacked would play their war-declaration cutscene
- Every other Cyrus would then congratulate you for declaring war on their fellow Cyrus
So much stuff like that happened all throughout the game, it was great.
I used to laugh at the "[Civilization] is planning to found a city with their new Settler" notifications, until I started receiving "[Civilization] has used a Settler to clear a Barbarian Camp" notifications. I guess you really can't take for granted that they're using their Settler to found cities.Notifications can be funny even in standard game.

Had a game as Cyrus on a desert island which had only me. Excellent and fun game until the AI civs found me with gunboats while I still had immortals and swordsmen lol.

Was the Sea level low? The land between two close continents should be shallow..
