Early/mid game crashing

Not sure where else to go with this. I've had Gods & Kings for a few weeks now and have completed a couple of games successfully.

My crashing started yesterday. I've attached a series of mdmp files - all have occurred in the last 24 hours. The one ending with 58C0 is from a new game started this morning - all others are from crashes and susequent auto-save reloads of the same game.

I've upgraded from Catalyst 11.12 to 12.6, but reverted back to 11.12 when the game simply fails to launch. In both the upgrade and downgrade, I did run the uninstaller first.

I've seen a couple of other suggestions to run the "repair" option on the "vcredist_x86.exe" that's packaged with the game. I've done this once.

I've also run "sfc /scannow" once. The output appeared to make a lot of adjustments.

Checking my Windows Event Viewer (yes, I have Vista :( ) I see "Application Error" events tied to my crashes. It appears that the faulting module is "MSVCR90.dll", with a "atidxx32.dll" that occurs about a minute in all my latest cases. I'm not convinced this is a video driver issue, given the first crash occurred before I performed the Catalyst upgrade and downgrade.

I've also run memcheck because I've had bad RAM before.

Any suggestions? The only common thread I see is that Attila and Ramses are in both games. I'll have to try a game without those two.

That error is indeed indicating driver and/or video hardware problems.

Any software installed recently that might interfere with the driver (e.g. hardware tuning/analysis tools?)

Already tried the 12.7 beta driver?

Is the video card stable when running Furmark?

Do crashes still occur if you run Civ5 on a single monitor?
Triple monitor gaming with only 1GB VRAM sounds a bit optimistic for me.

I find it odd that I've been running Eyefinity since the first week of April with little issues, even playing CiV (some minor slowdowns on larger maps). I've even played Deus-Ex: Human Revolution about 2/3 through with no issues.

If it is an issue with the video card, are we talking bad drivers, overheating, or something else?
 
If it's not a software problem, it could be anything, not necessarily the video card.
Mainboard, PSU, RAM, ...

Overheating is not very likely, but you could check this, too.
