I've been fiddling with the food-balance - I've found that doubling (almost) all food output, and increasing almost all food-costs by +50% has the effect I was going for (larger cities, chiefly.)
However, I can't do much to the health modifiers. Ideally, I would like to make each point of unhealth cost 2 food - failing that, I'd like each population point to generate 2 unhealth (and then I double all the health bonuses... you see where this is going.) Anyone know if I can do either of these things?
