Incidentally, I solved this by patching up to 1.74, then manually adding correct .dll files to my system. Just installing DirectX9.0C didn't work. Thanks for the help everyone gave me.

I reinstalled Civ IV, Patch 1.61, Patch 1.74 and the latest HOF mod but the problem still persisted. It turns out that the problem was caused by using an executable version of Civ IV that doesn't require the CD to be present. When I inserted the CD and used the normal .exe file, everything worked fine.
