I was having a similar problem just now. a HN Giant dared to enter my territory for killing a worker just near the boarder. He had orthus axe and was pretty strong but no match for my donal lugh. So i killed him off and expected the axe as reward but sadly I couldnt take it. It was still there at the end of the turn but still belonged to Rhoanna. Next turn the axe vanished. It was impossible for anyone to take that axe since I had no open boarders and HN Units must have killed my Donal before taking it. Im using the most recent patch...

You mean you were on the same tile as the Axe, but it still belonged to Rhoanna? If that is the case then that is where the bug started most likely.
