Please help this noob with city planning!

Okay, so I'm a new Civ player. I think I logged only like 50 hours in Civ 5. I'm 12 hours in and have lost 3 games.

The first game I lost, I didn't build enough combat units and got overrun by both barbs and the nearest civ.

The second game I lost, I was playing a duel as Rome against China. China tried to settle close to me and I wiped his city out and made him declare peace. Then, about 50 turns later I decided I wanted to end the game so I marched my army to his lands. Right before I declared war on him, he won a religious victory. I hadn't noticed his missionaries spreading Taoism in my cities.

The third loss I was America on a 4 player map. I made some units then rushed towards a religion so i could counter some of the religion spread that plagued me in the last game. Norway was close to me in the start and declared war early in the game. I ended up wiping him and taking his capital with the help of Russia. I built up to 4 cities and noticed Russia massing units near my borders so I declared on him and made him give up his second largest city. By this time, I had not met the final leader yet (Ghandi), as he was on a different continent. I had done a good job spreading religion to all of my cities as well as the nearby city states and St. Petersburg. Soon Ghandi's animated screen pops up and we exchange pleasantries. I suppose I got relaxed on my religion spread because I soon noticed both Washington and Nidaros were converted to Ghandi's religion and my other two cities were converted to Russia's! I ended up giving up because I was flustered and I had just been building stuff to build stuff.

So anyways, if anyone could give some helpful tips to this noob, it would be greatly appreciated. I never really know what to build, where to place which districts where, and what victory conditions to pursue. I really enjoy the game even though I have yet to win a game.
 
Maybe having a bare map of the early game city/surroundings could really help the discussion. Similar to how there used to be dotmaps in Civ 4, you can have really useful discussions on district layouts and city placement with a map at hand.
