IMO it is more important to pay attention to the change in score over time. On harder difficulties definitely plan on being behind for much of the game. However, once you are into AD years watch to see if you are gaining on the leaders. If the leader is increasing their score faster than you are gaining you may be in trouble, especially if one civ seems to be pulling away from the pack.

My rule of thumb is that if the game leader has double my score I'm likely screwed or at least need to consider desperate measures.

The problem with higher difficulty is often not that you are behind. Its that sometimes _one_ civ pulls very much ahead.
