In my most recent game, I've just noticed a 'red fist' next to the name of 'Sitting Bull'; two or three turns later he declared war - are the two things connected? I'm running 'BUFFY'.
 
The red fist doesn't necessarily mean the AI in question is preparing for war against you. And keep in mind there's a random "dice roll" element at work too, so the AI might prepare for war but not go through with it.

Uggg I have had this happen before. Roman wanted me to help him go to war with the native americans. So I said yes because it was on a different continent. I sent over 10 cats and 20 knights. He gathered a huge stack of units and just sat at the border for about 10 turns than decided to make peace with the NA and I got a hit with NA because I cut ties with it. But roman did like me.
 
The red fist indicates WHEOOHRN (We Have Enough On Our Hands Right Now) which appears in AI diplomacy when they're planning a war or fighting one. You can find it when you attempt to prompt them to declare war on someone - text shows as red and you can read the WHEOOHRN there. The mods make this easier to spot and is certainly convenient. The war plans can be against anyone with different likelihoods depending on the diplomatic situation, land accessibility and proximity - not just you.

There's no dice roll involved. The AI can voluntarily drop out of red fist only if they consider that their military buildup during the war planning is insufficient. Surprisingly the power of their intended target plays no role in this. Time scale is usually 10-20 turns, though it can be 1 or 200 in extreme cases due to various reasons.

that is their city count. If it is light blue, then you cannot see the trade screen and get an accurate count of his cities (AI will not talk, you are playing an OCC, etc) and the number is based on the city plots that you have revealed.
 
Keep in mind that some AI's can't plan a war against you if they are pleased with you. However, once they start planning, it doesn't matter how happy they are with you, if you were their intended target, that's not going to stop them.

Does it have something to do with trade? Sometimes it is positive and sometimes negative.
