The Spanish would have probably sent in another group. Of course, the Aztec (as well as their allies and their enemies) had been severely stricken with smallpox, so it would have been easier. The Tlaxcala might still have either allied with the Spanish or attacked them on their own, the Aztec were guaranteed to get weaker as a result of contact.

Since a lot of these things require extremely vivid imaginations, I'd rather focus on smaller changes (well, marginally smaller at the time).
What if the Americans, South Koreans, and UN soldiers made it to the Yalu River?
What if the North Koreans were able to overrun Pusan?
Maybe the only thing that changes is that all of Korea is the personal property of Lil' Kim, but it may have affected how much effort we put into further confrontations.

Well, Remus and Romulus were mythical, so their deaths wouldn't have meant much. If Rome never got founded, I expect the Greeks would have ended up having influence in a different way.
