When I played this as France I completely destroyed, I rushed the Great wall with 2 workers, then just sprawled out, taking most of spain, northern italy, nearly enclosed holland and bordered germany.
While that was happening carthage built a second city, had it taken by barbarians (who spawned axeman, but because carthage has no iron, couldnt defend itself), then that barbarian city became a seperate empire which then took carthage out, only to then be taken out itself by barbarians...
same thing happened to Vikings, who were Etheopia when i found them, but then lost the only two cities it had, also happened to england, who got taken by portugal and russia who didnt exist when I found it, so I dont know.....

So then I myself tried to add more civs to break the fog easier for the start of the game (first time trying any custom content for Civ4) and added,
Crimean Khanate - on Crimea, to fill in that area
Armenia - The people, not the country, based in central turkey
Khanate of Kazan - south of russia, north of crimean
Estonia - where estonia is, west of moscow
Kingdom of Sicily - on the island of sicily, gave it a galley instead of a scout, this was my fix to a lonely carthage
Aragon - also a fix for a poor africa, based in barcelona
Portugal - west iberia
Navarre - Between Paris and Madrid, France needs more competition when I played it as I dominated, having 20 cities before netherlands got 5
Brittany - another civ to compete with france, on its peninsula
Geneva - not a real civ, but needed something to fill in the alps, also added the lake for this civs start
Unfortunitly after fixing some of the resource gaps and adding all the custom city names it broke...
Doesnt work at all with the mod you supplied (as i intended) and I got it to work when playing as rome with normal BTS, but no custom civs showed up and half of the other civs dissapeared aswell...
