They all pretty much will backstab you. Alexander and Montezuma are the only certain wars, with France not far behind. The only difference is that Alexander actually tends to be good at war and have some reputable units. Monty invasions are just like swatting flies.

Siam and Greece.
Siam always backstabs. Always. It never fails. He is the most bloodthirsty and annoying leader in the game. If I don't go on the offense he pumps out elephants like there's no tomorrow.
Greece just all-out denounces me as early as possible. In my last game I started right next to him, and damn my starving population and rampant unhappiness I managed to completely conquer him by turn 30
Made the rest of the game a lot more peaceful!
Montezuma isn't so bad. He's annoying, but he likes to send those jaguar warriors on poorly-planned suicide missions, so he's not really a threat. He's also a lot more likely to ask for peace after you wipe out a few of his units, unlike most of these other guys who go for 1000 year wars.

In the first game of Civ V I ever played, I ran over Elizabeth and she has never forgiven me for it. My least favorite neighbors are the denouncers - Liz and Bismark in particular. They're very quick to denounce...and I am very quick to kill them.

I have had too many bad experiences with Gandhi, and I have even found him difficult to conquer after he gets his hands on Flight.
Catherine seems to be my least favorite neighbor...she expands very fast and I build my second city with water separating us and she instantly goes hostile. Fruitcake.
I actually have had great relationships with Monty and Khan. Not so much normally with Washington. He doesn't play well with others.

Pretty much any neighbor is going to be a headache, it just depends if they hate you more then they hate their other neighbor(s).
I especially hate having Genghis as a neighbor, he'll rapidly expand dropping near useless cities all around your borders just to block you in (drives me nuts) or he will DoW you repeatedly the entire game. I like to kidnap as many of his early unescorted settlers as possible to slow him down and get a free workforce. Suleiman seems to behave very similar. Genghis makes an excellent ally if you have a few civs between you.
Bismarck is a nuisance, always with an early denouncement just for the heck of it, usually followed by a DoW once he gets an army.
Isabella is an awful neighbor, constantly trying to invade. Even a few civs away she can be obnoxious like that. She seems to make a lot of friends and does make a good trade partner if she's far away.
Darius and Catherine are typically bad to have nearby, though Darius at least seems fairly honorable. I've had some excellent and long lasting relations with Darius after he failed to invade.
Askia has always been the single most untrustworthy leader in my games. If he lives close, he will back stab you, without fail.
Wu is usually reasonable, unless she thinks your an easy kill, then she'll back stab you, but so would most leaders. Shes one of my favorite neighbors. The same goes for Caesar and Al'Rashid.
And what is it with Gandhi and nukes? He's usually easy to get along with, but every once in a while he ends up taking over most of the map and eventually nuking all his neighbors.

I hate Spain, China and Russia as neighbours. (damn those girls!). Isabel puts on her smile all the time to squeeze the luxuries out from you for her money earned from exploration and then invades you when you have your army tied in another front. Wu never likes me even on first contact. Cathy is always friendly and tradey and when she DoWs, you have no time to prepare! 
I like Darius, Oda and Harun as my neighbours. Darius and Oda are honourable and when they get beaten, they become best friends. Harun is always there for luxury trade and rarely DoWs.

Just finished a game with Alex and Cathy as neighbours and I've grown to like them... on Prince. I enjoyed Alex beckoning me to go to war with him whenever I needed to pick up an extra city, then complaining about how great I was when he was unable to keep up.
Cathy was just fun to go to war with. I called war first, simply because my swordsman was stuck behind her borders, and ended up taking a city from her with it. You see, Rome had taken her city down to a couple HP, poised to take it the next turn. After I nabbed it from him, he just packed up and went home. Good Rome.
The other three wars with Cathy, she had better military and started it herself. I'd just, you know, basically sit there picking off her units until I had more soldiers, then charged in, took a city or two, gave her peace, and waited for the next war. Good stuff. I gave her lots of free luxs after I won to show there were no hard feelings, but she didn't seem too happy.

From what I've read, back in the days of the first game, civilizations acted randomly. When they had nuclear weapons, they would always end their diplo greeting with "My words are backed with NUCLEAR WEAPONS!". This became a meme when Gandhi would give a polite greeting followed by that. As a nod to this, Firaxis gave Gandhi a flavour of 12 out of 10 for nukes (meaning that even with the plus\minus 2 flavours, he will still always be 10 for nukes). He is also the only civ, as far as I know, who will actually say "my words are backed with nuclear weapons!" in Civ5 if he's hostile toward you.

Alex because of his annoying city state trait and his tendency to stay neutral the entire friggin game no matter what you do.
Bismarck is annoying. You build anywhere near him and he'll spam landsknechts EVERYWHERE and put fortify them on tile.
