Hello,
"sometimes" units in contaminated tiles suffers from a double -50HP effect; not a single -100HP, but a -50HP hover text appears twice and units are destroyed.
I found few mentions of this issue around, mostly here, here and here, where you can find even more details (mostly here) but problem seems still present
I've attached a savegame where units that will be destroyed are pinned on the map; you simply need to press "next turn" and they will die. Note that while tank is already damaged (but with more than 50HPs), the builder have full (100) HP.
In that save, a lot happens at beginning of next turn:
