Army Commanders - extremely vulnerable

I am. That's why I wrote it. :)

They return to your capital after a few turns of being KO'd. All promotions are intact. I don't think there is any way to permanently lose them. Even if they are "KO'd" during an age transition, they will be there for you at the start of the age.

Not only do they return but so do their stacked units. Some revolting peasants killed my commander. My view is they get demoralized and go on a drinking binge. There's a momento that reduces the time it takes to recover from their hangover.

"Packing" units into commander exists only for easier army transporting and moving troops between reserves and frontline. Your commander is not supposed to take hits.
Maybe I'm just crazy, but I've used the commander to tank hits a few times now. I prioritise ranged units early, and I rather my commander get hit than my ranged units, which would obliterate the enemy unit next turn.

I try to keep my commanders under one of the melee units attacking a city, to ensure that the experience is soaked up. The commander can tank hits in Antiquity, but the Modern units hit a lot harder. So far, I've been fighting relatively short wars (10 turns), so once peace is declared, I can have the commander and its packed units fortify until healed.
