Background-wise, we may assume that Nicolas did fall a victim to the assassination attempt in 1891 in Otsu, Japan, after which his younger brother George Alexandrovich became the
tsesarevich (heir-apparent) and eventually the Tsar. However, in 1899 he died of tuberculosis (as it happend in OTL), and was supposed to be succeeded by his brother Michael Alexandrovich. However, Michael was known to hate the idea of ruling a giant backward country, and in addition to this had the infamous "Romanov's curse" of haemophilia, so he managed to persuade his uncle Vladimir Alexandrovich, Alexander III's brother, to accept the
Monomakh's Cap (regalia of Russian Tsars) in order to prevent long-lasting interregnum and potential instability in the country that still remembered the crisis of Three Tsars of the early 19th century.
EDIT: Upon reading on Vladimir Alexandrovich's short bio, I consider also asking you to change his political ideology to moderate/centrist, primarily due to his personality:
