It's an invisible city.
How come the other two factions haven't founded a second city?
They tried at the start of the game, but Teutonic Knights stole settlers from both. Then I stole them from Teutonic Knights and after that, I destroyed them completely :P

You destroyed who? The Teutonic Knights or the Lithuanian/Volhynian nobility?

Don't they continue coming back though? It's been a while since I played this scenario.

They appeared twice or thrice before I got to them. However, after hunting every last unit and destroying their camp, they didn't come back. Three waves of Swedes came instead - two of which spavned next to my cites.

And about the Ottomans - I beelined to Winged Hussars and started to spam them. They came when I finished first Hussars. Before unlocking Hussars, I prepared two Crossbowmen. I spammed eight Hussars and I didn't need to care about them for the rest of the game - whenever they appeared, I used the Hussars + Crossbowmen and defeated them.
 
Messin round with the world builder seeing what the map generator does with different maps... currently rolling a few Pangeas just to see if I'd hate them, and I see this...

Imagine the epic battles that could happen if it happen to end up a border... a harbor at each end... as they fought over control of the center tile.

The map outline reminds me of a flying squirrel.
 
Yeah you can always settle within the normal city work range as long as the cities are on different landmasses. Note that landmasses actually means land separated by water this time, not the in-game Continents mechanic
 
Yeah, that's a map generation bug. There's a few mods that fix it. One or two specifically for the purpose, and YnAMP does as well.

Or you can wait for the patch that comes with the expansion, which will fix it as well.
