I try to find the difference but couldnt find it. It is about the terrain? what's the difference between the two: Start in old world and Start anywhere in game custom?

With start-in-old-world there will be at least two major continents. ALL civs will start in one of them (the old world), leaving a wilderness (well - with most game options it'll be full of barbs or barb-derived civs by the time you find it) to be discovered later in the game (the new world). The idea is that there is much more mid game colonization possible as a result.

Yeah, it is really much more fun that way. It should IMO be an option for all mapscripts, not just PW2/3, but I don't know how to do that.

Would not need it for Archipelago, Islands, or Lakes Maps.
