It definitly gives -25% city defense to all cities, but I dont think thats as simple as -25% to all units defending cities. It is factored in when city defense is calculated and I dont believe it can be negative (meaning being in a city wont ever be worse than standing on the plains). But it would effect the defense of the units if for example there were walls on the city or cultural defenses.
