I happened upon this picture of the "Formal Apology" form in New York State. If you have done someone wrong, you just fill this out and you're officially sorry, and I think it even counts in court. Anyway, for our purposes, you can make jokes with it!

I threw this one together with MS paint in about 3 minutes.

Thanks for that link, souron. I knew this was good, someone just had to start rolling with it. Note the top. "Attempts at humor are highly discouraged."

Even if you don't send anything, at least the picture is larger and easier to write on.
