Who is the only American to achieve five-star rank... Twice.
 
Arnold, once as general of the army, and then as general of the air force.

You could also argue that Ike, who was general of the army during WWII, and then restore to active duty as general of the army after his two terms as president can also fit the description of being 5-stars general twice.

The answer is indeed Henry "Hap" Arnold, General of the Army in 1944, and General of the Air Force (newly created) in 1949.
Ike doesn't count as he was just reinstated in the rank he already had.

Actually, it's Pierre Koeing, made maréchal in 1984 (Posthumus).

Well, the previous ones (Tassigny, Juin and Leclerc) were all made Maréchal de France in 1952, but Juin was the only one still alive (the two others were Posthumus).

But anyway, it's not the correct answer... As Maréchal de France is not the highest rank that existed during the history of France.
