Must have ancient walls

I've had a few instances where once I put up ancient walls in a city, I'll have barbs or another sim take a few shots at me. Why do the defenses never heal back up, and why can't I build later walls after that? City says it has ancient walls, medieval walls says I need ancient walls, what's up?
 
Scroll all the way to the bottom (should have put it at the top) of the build queue for the repair the defenses build. Until the walls are fixed you can't upgrade them.
 
Two different issues:

1. If walls are damaged, as long as they haven't been attacked for 3 turns, you should have a district project "Repair Outer Defenses." Walls do not heal by themselves. Until existing walls are repaired, you can't build the next generation of walls. (Same goes, by the way, for other buildings -- e.g., if your library is pillaged, you can't build a university in that district until the library is repaired.)

2. When you unlock the Civil Engineering civic, all of your cities automatically get +200 fortification and the ability to bombard. This civic renders obsolete all prior walls, but also means that damaged walls that weren't repaired before you get that civic can never be repaired. That would be fine, except the game appears to continue to present a grayed out option to build next generation walls, but only if your prior walls are repaired, which you can't do.
