I would doubt that NK or China would ever go nuclear against the US.
Outside of the Clancy-commie-conspiracy universe, in the real world, China far prefers competing economically, and NK is not suicidal. Neither of the leaderships is as manically mad and stupid as some would paint them (I say leaderships as there is more to NK than Kim Jong Il, and he seems on the way out within 10-20 years) They simply would not gain anything by it, and would lose the power they have.
I lean towards it being a non state group, but I hope and pray not. I reiterate that if, as it is increasingly suspected, Iraq has something to do with the anthrax situation, there may be some mushroom clouds in the desert, with weapons development sites taken out with a big symbolic gesture... I'm not saying it will happen, but it is a scenario that has a some small chance of occuring. The warning was made to Iraq in the Gulf conflict: Use WMD, and we will nuke you.
It remains to be seen what happens, but nuclear action by the US or Israel against Iraq is in my mind the most likely of a list of hopefully unlikely scenarios. It would have severe backlashes, even if completely justified, from the brainless hipsters of the lunatic Left, and the brainwashed masses of the Islamic world.
