Your last point is another possibility to solve the problem. I want to add an idea. How about starting a meeting of all leaders the killed civilization knew every time a king is killed, where the leaders decide who should get the remaining cities. You have to have at least one percent culture in the city to make demands. If you don't accept the demands you can declare war on the other fractions and the city rests independent.
Yea, but that's just kinda weird. If a civ is defeated, the winner isn't going to let everyone else take a piece of the pie. The most realistic would be the instant capitulation option.
This, of course, could be moderated based on civic choices, religions and maybe even technology.
For example:
Order and AV civs probably wouldn't capitulate if they lost their leader, they'd just elect/select a new one. This could be simulated with say, a 10 turn equivalent to stasis for Order and 10 turn anarchy for AV... and a temporary loss of leader bonuses for both.
Civs that can build immortals would logically grant their king immortality first. In this case, researching to the end of the melee line is a bonus in that your king will keep respawning.
Running the City States civic could be inherently unstable meaning that the death of the king could result in a certain percentage of the cities (is there a way to select only those that are near each other?) to split off into a new civ while the rest would instantly capitulate (very little loyalty, willing to sell themselves to a new leader).
If the civ is running Crusade at the time of their kings defeat, they can no longer switch off of crusade until the enemy is destroyed, the civ loses leader abilities but the crusade civic becomes more powerful (bonuses increase). After the enemy civ is destroyed, the Crusade civ has 10 turns of anarchy/stasis before a new leader is found.
