Shiggs Earth. I was playing France. Just took Rome. Met Carthage. Bought him out. 6 cities  , he has been busy. Got a lot of resouces. :king:

I've been allied with Militaristic City-states and helped them to capture cities that I didn't want. Whenever the CS advanced on an enemy city I'd destroy the enemy's units and bombard its defence to nothing, and the CS would easily capture it.

The most cities a CS has captured IME was 4.

In my current playthrough I was at war with Egypt and bribed a neighbouring CS so I'd have a foothold into Egyptian territory. The CS successfully captured the nearest Egyptian city, but the CS razed it and continued inland to try to capture more cities. I don't know if Civ5 has been changed so the CS' always raze cities instead of occupying them, or if this was just a special case -- regardless, it's interesting that the CS chose to destroy a city instead of keeping it for itself.

In an older version of my Earth map, Amsterdam went on a conquering spree:

I supplied the weapons and some supplemental bombardment. :D They obviously couldn't raze Paris or Roman, but I was pleasantly surprised when they held on to Sparta.

I once had Florence get up to roughly a dozen, they were the largest empire after me. True I was allied with them, had been supplying them with units to prosecute proxy wars on their side of the continent, had turned off city razing and this was back in the days when the AI loved to ICS and minimum city distances were lower. They ended up with nearly every Aztec city (except one I captured, two that I got another CS to capture and one that I allowed to survive), half the Japanese empire and a couple of American and French cities.
Protip: to get an allied CS to carry out a successful conquest choose an enemy city that is within sight range of units just inside the CS border, place a unit on the border then gift it to them. For extra efficiency supply your own artillery support.

I've seen City States conquer cities, but the one time one of my allies took over a city, a Japanese one I believe, they burnt it to the ground. I generally play on level 4 or 5, but I won on level 6. I think the new batches/fixes have made my style work better, but I'm kinda sad that I win mostly diplomatically. I've won all the different ways, but diplomatically seems the easiest to me, I don't even use Siam or Greece exclusively. Patronage is nice, but just gobs of money can buy an election.

Larges city state empire I've seen:

Current game: Indian AI allied with 18 City States at their peak. Large Archepello Immortal

I think you missed the point of the thread. It's about city states with large empires.

I've never seen a city state with more than two cities. Generally they don't wage war aggressively and when they do capture a city they almost always raze it. I have a hypothesis that they don't raze a city if it originally belonged to a civ they are allied with. No city razing is a totally different game of course, one I don't play.

I had a CS with five cities (four conquered CS nearby). Like previous posters, I stoked the fire by DOW on their neighbors and gifting them units to prosecute a proxy war just for the fin of in. In retrospect, I could have probably helped them to take two of the AI capital cities as well.

I'd post a screen shot, but I can't seem to get my screen shot function to work right. I posted a tech support question about the problem here.

I got the screen shot working. Here is a pic of Bucharest with five cities:

Neat! My best is an allied CS with three cities. Alex had DoWd the CS and I fed it advanced units. Worked well; I had a friendly place to land invaders on Alex's continent while he was chewing up his forces in attempting to recapture his lost cities. I DOWd and owned him a few turns later.
