Not necessarily my biggest failure, but still a pretty heavy facepalm occurred when I was playing Rhye's and Fall of Civilization today. I was playing as Spain and changed to Theocracy (and some other civics) on my starting turn so that I would be prepared for the inevitable spread of Christianity to my lands. It's 1500 AD now and still zero religion in my cities, what the heck? Suddenly, it dawns on me that Theocracy blocks out religions even when I have no state religion. Now I feel that I have to restart that game.
