The highest is probably somewhere in Western Africa, like Niger or Nigeria.

I'm not sure for the lowest, but most of the former USSR countries are negative.

Without cheating I'd say Indonesia as the highest and Germany as the lowest (although immigration might compensate for a low birthrate).
