Not always, but usually. I don't count that monster as a boss, it just didn't even come close to feeling like a boss so I never considered it to be one.

Suppose that you had the exact same level, except instead of green tentacles, spikes shot out at you every time you entered that room. Would it still be a boss, despite the fact that the change was entirely just cosmetic?

If it was spikes shooting out of a hole then it would be some kind of spike-shooter trap/obstacle.

IIRC, the guy doing Freeman's Mind had issues when recording in a Goldsrc (Which is why he is doing the segment in Half Life: Source instead of the Goldsrc version). So I doubt he'd do Blue Shift or Opposing Forces.
