The process is ridiculously simple. You go to Grand Cayman. They arrange transportation to Cuba. Your passport is not stamped but a stamp is placed on a separate document. It is extremely easy to do.

Note that I am not advocating but describing the rather boring way it happens.

Medical tourism is pretty big for America these days, I imagine that since this is a life or death decision most will simply travel regardless of the ban and simply emigrate. Better red then dead.

The only reason that diplomatic relations have not improved is Cuba's own bizarre government. Ask some escaping Cuban who makes it to the US shore about what goes on there.
 
Cuba doesn't stamp passport even for people who travel visa-free there. Because these people may have problems visiting USA later.
You are only getting stamp on departure, but it doesn't say where you are going to.

I spend two years ago some time in Cuba, and I would not believe everything escaping Cubans tell....

Cuba would be a perfect place for US pensionados to live :)
Just leave all the polarising ideological stuff behind you as well,
and enjoy life as it is, with BTW excellent healtcare for a few dollars,
A nice place to visit you for your kids and grandkids as well.
