For what's it's worth, I did run a visual comparison to Aethelflaed (who exists in the game as a Great General)

There are definitely some artistic liberties here and there, but the Devs always get the general clothing style right, at least. Needless to say, she could be Empress Maltilda or Eleanor of Aquitaine, but as of now, it's simply too early to draw any conclusions yet.
...it's also interesting to see how much slimmer they made Catherine in V. It's like the opposite of what happened to Teddy Roosevelt and Wilhelmina in VI. xD

I think this suggests that we will get natural disaster emergencies, obviously taking reference to the famous Pompeii and it's other distractions (sounds a little better than "Sid Meier's Civilization VI: Etna" but also more pronouncable then "Sid Meier's Civilization VI: Eyjafjallajokull) but I wondered if there could be another meaning for the name "Vesuvius"; of course the mountain it's self translates to "The one who lights the hearth"...
But otherwise; I wonder if we will actually get Mt Vesuvius in this expansion? I personally would prefer an "Amalfi" natural wonder with mt Vesuvius present in it (like Pulpit rock and Kjeragbolten are featured in Lysefjord)
So I was doing more digging (this time looking into Consorts rather than rulers), and I was wondering if the "concept art" could be related to this individual, Margaret of Anjou. There's some similarities in their appearance.

She's be a very weird pick, though. Consorts rather than rulers in their own right are controversial, to say the least.
Just quoting to say I did my doctorate on the manuscript that image is from and so find it so odd to see it here
But like you I doubt very much that Margaret/Marguerite would make an appearance as a leader. The leaked image also looks twelfth/thirteenth century to me as opposed to fifteenth. I'd love it to be Eleanor of Aquitaine but alas that seems just as unlikely!
