Wine has the advantage of being around in Western culture for a lot longer than either tobacco or opiates, though (excluding the medicinal use of poppies, of course). Wine is something that has been regularly drunk by just about every Western society since antiquity - sometimes as an alternative to undrinkable water. It has the whole Biblical references thing going for it, too - if the Bible talks about Jesus drinking wine, it's probably OK, right?
Another thing to remember is that the advertising of tobacco is a lot more restricted in many countries around the world than the advertising of wine (in the UK, we've been discussing the possibility of removing manufacturers' names on cigarette packaging, for example, in addition to the health warnings you get plastered across every packet). I could see that adding tobacco would affect the game's rating in many countries, whereas with wine, not so much. Wine is still viewed as more culturally acceptable and doesn't carry the same stigma that tobacco or opiate narcotics do (although some would argue that perhaps it should...)
