My favourite.

This is one I remember from a similar thread elsewhere. A TV series rather than a book though.

#34. One of These Things is Not Like the Others.

This series, of which only the pilot was shot, is the greatest error in the history of Jim Henson's Creature Shop (Farscape excluded). In Henson's defence, however, hate crimes against minorities were "less prevalent" in the original script, and top executives mistakenly believed that the show was sponsered three times over by the letter 'K.'
