What exactly is a metropolis?

Size 1 to 6: town
Size 7 to 12: city
(requires either being directly adjacent to a source of fresh water, or having an aquaduct)
Size 13 and up: metro
(requires either the Shakespears theater great wonder or a hospital)

Not really. A single size 20 metro needs more happiness/contentness than 2 size 10 cities, not to mention the upkeep of the hospital itself, and maybe the upkeep of the mass transit, or the worker gang you have to keep around to clean up pollution. 2 cities also give more free unit support than a single metro.

An empire of metro's is an awesome sight though.

A Civ 3-metropolis is a citylevel that has got the setting for its size and its name in the general settings in the editor.

For the vanilla game, these settings are as it has been said (size 13 or bigger and that name for level three of a city). But of course the necessairy sizes to reach the next level for a civ 3 city can be changed in the editor as it can be the name for that city level.

The citylevel three (metropolis) is a perequisite to suffer from population pollution. So p.e. in my civ 3 epic mod the size to reach city level three is set to 999 and therefore in reality there is no city level three in that mod.
 
You will make more money from a set of size 20 Metros than a set of size 12 cities holding the same area, because corruption will be reduced, and, of course, you can add in more specialists if you want, plus you have fewer improvements to worry about.
 
Assuming you have all 8 lux resources, and don't have WW to worry about Otherwise you end up needing to build temples cathedrals and colloseums too. Thats 3 extra improvements, worth 5 gpt. And you may also want to build a mass transit.
