By the way, I've had a very similar conversation with Ciceronian in real life about a very similar but parallel issue. It concerns debt cancellation and extending a hand to the third world. Cicer reckons it is naive to do so because they (like the enemies whose life we would save in this scenario) have instincts to dominate just like anyone else. In helping the downcaste you set up your subjugation in times to come. Well this is cynical, selfish, computer game driven claptrap, which frames the world in the context of dominance and subjugation alone and it denies the quite natural human urge to help one another as a cohesive group. This does indeed caste the compassionate as naive - but perhaps the compassionate would rather be naive given the options.

I bolded the brilliant part. Judging from this,it seems like being naive is something recommendable.
And to the topic; I would save all three.

Edit: Besides, it can't really be "murder" can it? I'm not at all well-informed in criminal law, but I doubt doing nothing means "murder". At least not in Sweden I guess...

That's the option I was looking for, None of them.

So, if I had to choose, I'd pick my worst enemy to save, and then kill him after he thanked me for saving him.

I'd save them all. I even save my worst enemy because letting or causing someone to die is, in my view, not a way to deal with conflict under any circumstances.

The reaction of my worst enemy to me saving his life along with the original reason he is my enemy would govern how I would deal with him after the fact.

You are very right, I should have included a way for you to forsake them all, but I think you could vote the last option for that... But why would you want to kill them all?

I would save them all - my best friend for obvious reasons, the stranger because I'm not a cold-hearted person, and my worst enemy because my worst enemy isn't all that bad. However, if I had a worst enemy that I truly, truly loathed above everything else (probably only for attacking my family), I would probably let him die - but it's really hard to tell because I've never hated anyone nearly that much, and I've never really been in a position in which I could choose whether to help an enemy or not.

I would save my friend and the stranger. I don't have a worst enemy but if I did, it would surely be quite a horrible person (one capable of murder, torture, rape, etc.) so while I might not actually kill him, I certainly wouldn't save him.

This isn't exactly murder, there's nothing that says that the disease is terminal. Rather, this is just allowing them to be tortured mercilessly for years.
