To get Brian, you need to go out back, go through the forest, and try to warn to Courvers (I think that's what their name was). He is the head of that household. He is also very independent and I am curious as to whether you can save anyone else once you grab Brian and stick with him.

I ran into Brian, saved Justin and Kayden, and then headed for the mall. I sided with Brian, and left Justin and Kayden at the mall. Brian and I were ambushed by zombies, but Brian saved me as he died. Then I headed towards the Island, where I fortified the house. Alas, the zombies got past my traps and my gun, and overwhelmed me, killing me.
Rest in peace, Brian.

I survived the game. I didn't discover the "secret" of the island, but I managed to hold off the final zombie horde. Brian and I made for a great team -- independent, feisty, "don't take crap from anyone". We went to the mall, left when they tried to take our guns (good choice), leaving Justin there to die. I saved Brian in the alley, took the speedboat, and left the baby to die.
Is there any other way to win without going to the island?

Well, technically, you always "win" even if you don't survive. I mean, in one of my games, I died, but I saved four people, sacrificing myself. I'd say that's a victory.
I also believe that there's something else you can do with the Island other than kill all the zombies. I'm not sure how, though.

Well this just got too may replies for me to ignore.
I did this, essentially. Except Bonnie (Female Brian) left me at the mall. I escaped the area with Kayden and Jennifer/Justin, but after five playthroughs I was sick of her whining about how much Micheal/Michelle hated her and I threw her to the zombies. Needless to say, this startled young Kayden who ran away from me. I made it to the island alone, but from an earlier playthrough I had figured out what I needed to do to make it. I didn't save anyone, but I survived.

Has anyone survived by staying at the church/mall, or is the island the only way to win?

I don't think you can:
#1: Went to the Church. Died fleeing that crazy dog Anita. (If you play through it, it's pretty clear you can't stay there)
#2: Went to the Mall, helped out with the first thing [can't remember what it was] waited until it was overrun and escaped. Died on the island w/ Justin in the cellars.
#3: Went to the Mall. Yelled at Anne and left. Died saving him on the road. Followed the same path as before, so skills must have played a part somehow.
#4: Went to the Mall. Died helping that useless brat Kayden. (Didn't help out w/ fortifying. With that, I'm pretty sure you can't stay at any of the three locations because you can't make it to prison.)
#5: Went to the Mall and helped out with the second thing, also can't remember what it is. Survived. Alone, but I'm pretty sure if I had picked a certain choice in #2 I would have made it.
I don't know why these things aren't more popular.

#6: Got everyone I could, basically gave Anita the finger when she asked for my stuff and then she left me to die. Yeah...has anyone managed to survive after arriving at the Church?

This site also has other, similar games to play with. I for one reconnmend the vampire one for having an extremely fluid storyline that is interesting.
