Considering a Milan governor rules in Genoa, they really don't have a reason to invade you as all they need to do is replace the governor whenever they want.

Only if you let him. Pay attention to the factions in your nation (their loyalty and their strength) You can spend money to gain loyalty and use your power to lessen that of those who oppose you.
