I have been playing this mod for a while (currently at 18th century and in industrial era more or less) and its really good but there are some things I would like to comment on.
1) early game: limited options to build practically forces you to expand, but to build army big enough to kill a barbarian city requires you to gimp your economy for quite a while, before you lose 90% of the army getting the city. A bit irriating as at some point all you can do is build units and delete them.
2) civics - I think that the method with only one civic to chose from is rather limiting. Probably the biggest global gripe I have with this mod so far.
3) preset relationships - don't like this at all (second biggest gripe). It makes things predictable and also allows to easily conquer friends without penalty. I started as canada, taken over US, Mexio, Brasilia now Im going to go after Argentina and thanks to preset standings with (+25 is really well enough) I can ignore everyone and they dont seem to care about my warmongering.
4) it seems AI is generally not aggresive enough. After quite long game sill about 7 barbarian cities in northern asia are not conquered and no civilization has actually become significantly better then others. Which is a shame, as I expected China or Russia to do some decent landgrabbing.

5) Medic promotion seems to be not working. I haven't seen any unit type since I researched Microscope until Riflemen/Grenadier/Cuirassier that can be upgraded to Medic
