For me it probably was Civilization II. I must have played that game for a decade. Since i am done playing games, it will be the top one for me.

Honorable mention to Europa Universalis II, which i still played up untill three years ago.

Morrowind, Civilization II, Age of Empires II (and expansion), Stronghold, Call of Duty: United Offensive, Realmz, Team Fortress 2, Rome/Medieval2/Empire Total War combined (each is over 100 hours), Mount&Blade, Oblivion (though Dragon Age and Bad Company 2 are catching up). Possibly a couple of others from the late 1990s.

As for the one I've played the most, I do not know. I played a LOT of Morrowind, but also nearly 300 hours of TF2 (which isn't a lot, most of the people whom I've played with for a year+ in TF2 had long ago passed 300 hours).
 
I think the PC game that I played the most was SimCity 2000. It was the first PC game that I got when I got my first PC, and then bought the Special Edition a few years later when the disc for the original was broken.
 
Diablo 2, probably, long time ago, Civilization IV too. World of Warcraft, a couple of years ago.

I spend a LOT of time playing games, but these days, I have too many of them to spend a lot of time on a single one. I think this is actually kind of sad in a way. I feel jaded by all these games and few of them manage to catch my attention very much for over 40 hours. Back in the days, I bought a game or had my parents buy one and, well, I just had to spend months playing it 'cause that's all I was gonna get for all this time...
 
Dark Age of Camelot is first by a looong shot, Master of Magic, CiV II, Diablo 2 are runner ups (runners up?).
 
WoW hands down. I don't play video games that much, but that and Mario Kart are the only ones I've ever really been obsessed about.
 
tf2 or civ4...

also got quite a lot of time on MW2 and cod4, but probably less than half what i have on those two.
 
Ummm...Age of Empires 2, Age of Empires 1, Rome: Total War, (though I'm loathe to admit it) Dynasty Warriors 3 and 4, Medal of Honor: Frontline, Civ4, Ricky, Half Life, Halo, Mass Effect

Those are probably my top 10 in order of time spent playing it.

I think I've beaten Deus Ex more than any other game, but I dunno what I've spent the most time on.
 
Can't say if it's Starcraft or Civilization in total. Starcraft original/BW probably beats any civ title, I don't know what exactly to count under civ3 + civ4 combined and related things like these forums but it could be making a run for it. - haven't played much SC2 in the scheme of things of cours.

tf2 is not up there at the top, I know that. But I have 200-300 ish hours I think and absolutely love that game. About the only shooter I've ever played any large amount of though. I think smash bros melee might be around third total, and the series altogether especially if brawl is added onto that.

I'm also doing very unreliable math right now I know, because as a kid it's hard to judge how much you did and while over many years I played a lot of games it also wasn't generally for long times at all - 30 minutes a day at most as a young kid during some break/holiday/summer actually doesn't match just playing for 3-4 hours on a weekend like I might commonly do sometime now. (and particularly for an individual game, as there are only a few I commonly play now so all the time goes to those same totals)

I've probably got a longer list of games with the 50ish hours/significant playtime cutoff over many years. Most recent apart from those listed above is probably Nethack which quickly rose to be one of my most awesome games ever.
 
Fallout 2, Counter Strike and Command and Conquer Generals.

Civ 3 ain't far behind.
 
Unsure. Spent a helluva lot of time playing Mass Effect and Mass Effect 2. Mario Kart 64 is probably up there, along with TimeSplitters 2. Team Fortress 2 I've spent at least 200 hours playing, back when I still played it. Command and Conquer: Red Alert 2 and Rome: Total War are both in the running, too - Yuri's Revenge was awesome, and Europa Barbarorum gives me hope in humanity. To round it out, I loved playing Ace Combat 04: Shattered Skies.

If I had to guess, I'd say Mass Effect, but it's in flux. Now that I have all of the save files I need, I doubt I'll play any more of that game from here on out, and Mass Effect 2 will probably replace it soon.
 
Civilization III for a very long time. HALO since the first one launched way back in '01, Age of Empires, the Elder Scrolls III and IV, I have not had the honor of playing Daggerfall, and the Total War series.

Lord of the Rings Online back in the day (January 2008) to immediately before the Siege of Mirkwood. There's a long story there over that year and eleven months, which is the longest I've played an MMORPG without intermittent periods.

World of Warcraft for three years (2007 to 2010) with periods of break in Lord of the Rings Online.
