Some questions I can't find the answers for

I think it is guranteed if reqs are met.
That said, there's a heap of things that can affect wltkd.
If pop is 6 in the one town and 5 in the other on the turn you checked, one might have wltkd while the other will not (this may or may not be news to you, I'm just pointing out another of the many things that can affect wltkd).
 
In the "Can someone clear up the FP thing?" thread, a couple threads in there hint that building courthouses and police stations not only decrease corruption and waste in the city where they're built, but also marginally increase the OCN.

Is that correct? If so, that would explain what I was seeing. As other cities in my empire build Courthouses and Police Stations, the OCN moves gradually upwards, thus eventually providing less waste and corruption for older (even possibly 1-turn older) cities.
 
No. They increase the OCN for the city in which it is built. Each city has it's own modified OCN and it is this modified OCN that is used as part of calculating the individual city's corruption/waste.

Forbidden Palace is different. It increases the OCN for all cities in addition to helping corruption/waste in the city in which it is built as well as serving as an additional center for distance corruption.
