Czechoslovakia, Latvia/Lithuania have merged and grown, Bulgaria has grown, Vietnam has absorbed Laos and Cambodia.
Ethiopia has absorbed Somalia and Erithera. Congo has absorbed Uganda, Rwanda and Burundi. Georgia, Armenia and Azerbaijan have merged.

We get twice as many Germanies and the Low Countries are finally reunited, as is Ireland.
Tolni is in heaven.
The Baltics are now one, ending the name confusion everybody else has about them once and for all.
Puerto Rico's now officially part of the U.S. of A.
Saudi Arabia has put an end to the pesky little emirates on its eastern buffer zones.
Israel has finally conquered the whole of Jordania.
No more trouble in the Caucasus.
Harf of Karafuto is now back in hands of grorious Nippon.
Zanzibar's been sunk under the waves.
The Horn of Africa is a mess.
Viet Nam has conquered the land of Laos, possibly under Chinese auspices.

The Balkan change is that it's now Bulgaria + (North) Macedonia + Western Thrace/Chalkidiki (from Greece) + Leskovac/Pirot (from Serbia). Slightly larger than the borders of the Bulgarian occupation zone during the Second World War.
It's Benin + Togo and China + Nepal + Bhutan. You miswrote Gambia as Gabon.

The inter-Sudan border has changed.
Czechia + Slovakia.
Romania + Moldova.
Gabon + Equatorial Guinea.
Kenya + Uganda. Tan(zan)ia + Rwanda + Burundi.
Mozambique + Malawi.
Botswana + Caprivi Strip.
Hong Kong is apparently independent.

Austria + Switzerland may be a thing (or it may be an artifact of low world map resolution).
There is a new island in the Eastern Mediterranean.

Several large islands and island nations are missing and others are bizarre-looking, whether due to low map resolution, incompetence, or malice. Northern Canada is all kinds of hosed-up. The Falklands/Malvinas are gone. Comoros, Seychelles, Mauritius, and Maldives are all missing. The Melanesian islands are mostly there, but the Polynesian and Micronesian islands are not. This is all in sharp contrast to the Antilles, which mostly look like they're there (less some of the Leeward Islands and Caymans).

At first I thought it was due to map resolution as well but if you look closely they managed to get Vatican City in there so it was probably intentional.

As for the island in the Mediterranean, I assume that it's Atlantis risen again.
