I'd say a lot of them are completely accidental, and the rest is made by stupid people whose job is to make pretty graphics, not know anything about what it represents. What's more annoying is that those whose job it is to spot these mistakes and correct them aren't doing what they're supposed to do. This is especially bad when it happens to reputable newspapers like NY Times.

I also hate all the maps which focus solely on the Eurozone and ignore the rest of the EU completely, as if they were two totally separate entities.
Another thing, every map with an independent Catalonia I see has Valencia and Baleres independent too. Well, there are minors independence movements in Catalonia, but Valencia and Baleares don't want to be independent, I mean, there are no movements/organizations/parties or anything fighting for the independence.
That depends if you're talking with an pan-Catalanist independentist or not. BTW, what is that map supposed to mean?
